Physical Therapist- Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform mobility, ROM, transfer, balance, and safety tests.
• Implement wound care and dressings as needed.
• Develop and adjust PT plans (manual therapy, gait training).
• Continually assess and revise care plan with nursing.
• Educate clients and families on rehabilitation, safety and independence.

🎯 Requirements

• Current Pennsylvania Physical Therapist license.
• Minimum one year of PT experience.
• Graduation from APTA-approved program.
• Ability to work independently and manage time.
• Strong interpersonal and organizational skills.
• Solid computer skills; EMR experience preferred.
