- Make home visits to clients in designated territories.
- Perform mobility, range of joints, transfer status, stability, ambulation, strength, balance, and safety assessments.
- Implement integumentary protective interventions to prevent wounds and promote repair using evidence-based dressings.
- Develop individualized care plans including manual therapy, gait training, balance, and other interventions.
- Continually assess and revise the PT care plan; participate with nursing in multidisciplinary care plans.
- Educate clients and family members in rehabilitative care and activities; document in EMR with a touchpad tablet.
π― Requirements
- Current Pennsylvania Physical Therapist license.
- Minimum of one year of recent PT experience.
- Graduate from an APTA-approved program or AMA/CAAHEA-accredited program.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem solving skills.
- Solid computer skills; EMR experience preferred.
