Physical Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform mobility, range of joints, transfer status, stability, ambulation, strength, balance, and safety assessments.
• Implement integumentary protective interventions to prevent wounds and promote repair using evidence-based dressings.
• Develop individualized care plans including manual therapy, gait training, balance, and other interventions.
• Continually assess and revise the PT care plan; participate with nursing in multidisciplinary care plans.
• Educate clients and family members in rehabilitative care and activities; document in EMR with a touchpad tablet.

🎯 Requirements

• Current Pennsylvania Physical Therapist license.
• Minimum of one year of recent PT experience.
• Graduate from an APTA-approved program or AMA/CAAHEA-accredited program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
