Physical Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests: mobility, ROM, transfers, strength, balance, safety.
• Implement wound care to promote healing with evidence-based dressings.
• Develop and revise individualized care plans including manual therapy, gait, and balance.
• Educate clients and families to promote health, safety, independence.
• Accurately document observations and interventions in EMR.

🎯 Requirements

• Maryland PT license.
• Minimum 1 year PT experience.
• Graduation from an APTA/AMA-accredited program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficient computer skills; EMR experience preferred.
