Physical Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements: mobility, transfer status, strength, balance.
• Implement wound care and integumentary interventions using evidence-based dressings.
• Develop care plans with manual therapy, gait training, balance.
• Continuously assess and revise PT care plans; coordinate with nursing in multidisciplinary plan.
• Educate clients and families in rehabilitative care to promote health and independence.

🎯 Requirements

• A current Maryland Physical Therapist license.
• Minimum of one year of PT experience.
• Graduate from an APTA-approved program; transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
