Physical Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Assess mobility, ROM, gait, strength, balance, safety.
• Provide wound care with evidence-based dressings.
• Develop and update individualized PT care plans.
• Educate clients and families on rehab and safety.
• Document observations and interventions on a tablet EMR.

🎯 Requirements

• One year of clinical PT experience required.
• Maryland PT license required.
• Home health and Oasis experience required.
• Graduate from APTA-approved program.
• Strong interpersonal and organizational skills.
• Ability to work independently and manage time.
