Make home visits to clients in designated territories.
Conduct mobility, ROM, gait, strength, endurance, balance, and safety assessments.
Implement wound care with evidence-based dressings.
Develop individualized PT plans (manual therapy, gait training).
Collaborate with nursing on multidisciplinary care plans.
Educate clients and families on rehab and at-home safety.

🎯 Requirements

Maryland PT license.
Minimum 1 year of recent PT experience.
Graduate from an APTA-approved program.
Ability to work independently and manage time.
Strong interpersonal, organizational, problem-solving skills.
Solid computer skills; EMR experience preferred.
