Physical Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests: mobility, ROM, gait, balance, strength.
• Implement wound care and protective interventions as needed.
• Develop and revise individualized PT plans: manual therapy, gait, balance.
• Document observations and interventions in EMR via touch-pad tablet.
• Educate clients and families on home care and safety.

🎯 Requirements

• Maryland Physical Therapist license.
• At least 1 year of PT experience.
• Graduation from an APTA-accredited program or CAAHEA.
• Ability to work independently and manage time effectively.
• Interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
