Physical Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements: mobility, ROM, transfer status, stability, ambulation, strength, endurance, balance, safety.
• Implement wound care interventions with evidence-based dressings and topical agents.
• Develop and implement individualized care plans, including manual therapy, gait training, balance, and other interventions.
• Continuously assess and revise the PT care plan with nursing in the multidisciplinary team.
• Educate clients and families in rehabilitative care to promote health, safety, and independence.

🎯 Requirements

• A current Maryland Physical Therapist license.
• Minimum of one year of recent PT experience.
• Graduation from a program approved by the American Physical Therapy Association or AMA accreditation.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior EMR experience preferred.
