Physical Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests and measurements (mobility, strength, balance).
• Implement protective wound care using evidence-based dressings.
• Develop and implement care plans with manual therapy and gait training.
• Continually assess and revise the PT care plan with nursing.
• Educate clients and families in rehabilitative care for health and safety.

🎯 Requirements

• A current Maryland Physical Therapist - PT license.
• A minimum of one year of recent work experience as a Physical Therapist - PT.
• Graduation from a program approved by APTA/CAAHEA/AMA (transcript or diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
