Physical Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform mobility and strength assessments (ROM, balance, gait, transfers).
• Implement wound care and protective interventions with evidence-based dressings.
• Develop and adjust individualized care plans (manual therapy, gait, balance).
• Educate clients and families on rehabilitation and safety.
• Document observations and interventions in EMR on a tablet.

🎯 Requirements

• A current MD Physical Therapist (PT) license.
• Minimum 1 year PT experience.
• Graduation from an APTA/CAPTE-accredited PT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
