Physical Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Conduct diagnostic tests and measurements (mobility, ROM, balance, gait, safety).
• Implement wound care and dressings to promote healing.
• Develop and revise individualized care plans (manual therapy, gait training, balance).
• Educate clients and family on rehabilitative care to promote independence.
• Document observations and interventions in the EMR using a tablet.

🎯 Requirements

• Current Maryland Physical Therapist license.
• Minimum of one year of recent PT experience.
• Graduation from APTA-approved program or AMA accreditation.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior EMR experience preferred.
