Physical Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements: mobility, ROM, gait, balance.
• Implement wound care and protective interventions with dressings.
• Develop and revise individualized PT care plans with exercises.
• Educate clients and families on rehabilitation and home safety.
• Document observations and interventions in EMR.

🎯 Requirements

• Active New Jersey Physical Therapist license.
• Minimum one year PT experience.
• Graduation from APTA-approved program.
• Ability to work independently and manage time.
• Strong interpersonal and organizational skills.
• EMR experience preferred.
