- Make home visits to clients in designated geographic territories.
- Perform diagnostic tests and measurements (mobility, balance, strength, gait, safety).
- Apply wound care and dressings using evidence-based therapies.
- Develop and implement individualized PT plans including manual therapy, gait and balance.
- Continually assess and revise the PT plan; collaborate with nursing in the multidisciplinary care plan.
- Educate clients and families on rehab for health, safety, independence.
🎯 Requirements
- Maryland PT license.
- Minimum 1 year PT experience.
- Graduation from an APTA/AMA-accredited PT program.
- Ability to work independently and manage time.
- Strong interpersonal, organizational, and problem solving skills.
- Solid computer skills; EMR experience preferred.
