Physical Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests and measurements (mobility, ROM, balance, gait, strength).
• Implement wound care and protective interventions with evidence-based dressings.
• Develop and revise individualized care plans including manual therapy and gait training.
• Continually assess and revise the PT care plan with nursing in the multidisciplinary team.
• Educate clients and families to promote health, safety, and independent living.

🎯 Requirements

• A Maryland Physical Therapist license.
• Minimum of one year of recent PT experience.
• Graduation from an APTA-approved program or AMA CAAE program as indicated by transcript/diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior EMR experience preferred.
