Physical Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of mobility, strength, balance, and safety.
• Implement protective wound interventions using evidence-based dressings and topical agents.
• Develop and implement individualized care plans including manual therapy, gait training, balance.
• Continually assess and revise the PT care plan with nursing in the multidisciplinary team.
• Educate clients and families in rehabilitative care to promote health, safety, and independence.

🎯 Requirements

• Maryland PT license (current).
• Minimum of one year of recent work experience as a Physical Therapist.
• Graduation from APTA- or AMA-accredited program (per transcript/diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
