Physical Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform mobility, ROM, transfer, balance, strength, endurance, and safety assessments.
• Implement wound care using evidence based dressings and topical agents.
• Develop and implement individualized care plans including manual therapy and gait training.
• Continuously assess and revise the PT care plan with nursing as appropriate.
• Educate clients and families to promote rehabilitative care and independent living.

🎯 Requirements

• A current New Jersey Physical Therapist (PT) license.
• Minimum of one year of recent PT experience.
• Graduation from an APTA-approved program or AMA CAAHEA-accredited program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
