Physical Therapist- Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements: mobility/ROM, transfers, balance, strength, and safety.
• Wound care and prevention using evidence-based dressings and topical agents.
• Develop and implement individualized care plans, including gait training and balance.
• Assess and revise the PT care plan with nursing in the multidisciplinary team.
• Educate clients and families in rehabilitative care for safety and independence.

🎯 Requirements

• A current Pennsylvania Physical Therapist - PT license.
• A minimum of one year of recent work experience as a Physical Therapist - PT.
• Graduation from a program approved by APTA or AMA CAAHEA, as shown on transcript.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior EMR experience preferred.
