Physical Therapist- Home Health Visits

📋 Description

\n
• Make home visits to clients in designated territories.
\n
• Perform diagnostic tests and measurements (mobility, transfers, balance, strength).
\n
• Implement wound care interventions with evidence-based dressings.
\n
• Develop and revise individualized care plans (manual therapy, gait, balance).
\n
• Assess and revise the PT care plan with nursing in the multidisciplinary team.
\n
• Educate clients and families on rehabilitation and independent living.
\n

🎯 Requirements

\n
• A Pennsylvania PT license.
\n
• Minimum one year of PT experience.
\n
• Graduation from an APTA/CAAHEA-approved program.
\n
• Ability to work independently and manage time effectively.
\n
• Strong interpersonal, organizational, problem-solving skills.
\n
• Solid computer skills; EMR experience preferred.
\n
