Physical Therapist- Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and mobility assessments (range of motion, balance, gait, strength).
• Implement wound care and protective interventions with evidence-based dressings.
• Develop and implement individualized PT care plans (manual therapy, gait, balance).
• Educate clients and families on rehabilitative care and safety for independence.
• Document observations and interventions in the EMR.

🎯 Requirements

• Current Maryland Physical Therapist license.
• Minimum of one year recent PT experience.
• Graduation from an APTA-approved program or CAAHEA accreditation.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
