📋 Description

• Make home visits to clients in designated territories.
• Perform mobility, strength, balance and safety assessments.
• Develop and implement individualized care plans and therapeutic exercises.
• Educate clients and families on rehabilitative care and safety.
• Document observations and evaluations using EMR tablets.
• Collaborate with nursing and multidisciplinary team.

🎯 Requirements

• Current Physical Therapist (PT) license.
• Minimum one year of PT work experience.
• Graduate of an APTA/AMA-accredited program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
