📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements: mobility/ROM, balance, safety.
• Implement wound care using evidence-based dressings and topical agents.
• Develop and implement individualized care plans: manual therapy, gait, balance.
• Collaborate with nursing in multidisciplinary care plan updates.
• Educate clients and families to promote health, safety, independence.
• Document care observations and interventions in EMR.

🎯 Requirements

• Current NJ Physical Therapist (PT) license.
• Minimum 1 year PT experience.
• Graduate from APTA- or CMAA-accredited PT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
