Physical Therapist, Home Health

📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests and measurements: mobility, ROM, transfers, balance, strength.
• Implement wound care using evidence-based dressings and topical agents.
• Develop and implement individualized care plans including manual therapy, gait, balance.
• Continuously assess and revise PT care plans and coordinate with nursing on the multidisciplinary team.
• Educate clients and family on rehabilitative care to promote health, safety, and independence.

🎯 Requirements

• Active and current PA license to practice as a Physical Therapist (PT).
• Home health experience welcomed, but training provided.
• Ability to travel to assigned cases.
• Strong communication, assessment, and documentation skills.
• Compassionate, patient-centered care focused on home-based services.
