📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements: mobility, transfers, balance, safety.
• Implement wound care using evidence-based dressings and topical agents.
• Develop and implement individualized care plans with exercises, gait, balance.
• Assess and revise the PT care plan; coordinate with nursing.
• Educate clients and families to promote health and independence.
• Document observations and interventions on a tablet.

🎯 Requirements

• Active and current PA PT license.
• Home health experience welcome; training provided.
• Ability to travel to assigned cases.
• Calm, reassuring presence; strong listening and communication.
• Compassionate, reliable care for patients.
