- Make home visits to clients in designated territories.

- Continually assess and revise the PT care plan, and coordinate with nursing.
- Educate clients and families in rehabilitative care and document observations using EMR.
π― Requirements
- One year of clinical experience as a licensed Physical Therapist is required.
- Current New Hampshire PT license is required.
- Graduation from a program approved by the APTA or AMA Committee, as indicated by transcript/diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem solving skills.
- Solid computer skills; prior experience with EMR preferred.
