Physical Therapist- Home Health

📋 Description

• Make home visits to clients in designated territories.
• Perform mobility/ROM, transfer, balance, strength, endurance, and safety assessments.
• Implement wound care using evidence-based dressings and topical agents.
• Develop and implement individualized care plans, including manual therapy, gait training, and balance interventions.
• Continually assess and revise the PT care plan, and coordinate with nursing.
• Educate clients and families in rehabilitative care and document observations using EMR.

🎯 Requirements

• One year of clinical experience as a licensed Physical Therapist is required.
• Current New Hampshire PT license is required.
• Graduation from a program approved by the APTA or AMA Committee, as indicated by transcript/diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with EMR preferred.
