📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements: mobility, ROM, transfer status, balance, strength, and endurance assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair with evidence-based dressings and topical agents.
• Develop and implement individualized care plans, including manual exercises, gait training, balance, and other interventions.
• Continually assess and revise the PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct clients, family members, or other representatives in rehabilitative care to promote health, safety, and independent living.

🎯 Requirements

• Current FL PT license.
• At least 1 year PT experience.
• Graduation from APTA-approved program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
• Florida Background Screening Clearinghouse.
