📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, ROM, balance, gait, safety).
• Implement wound care and dressings using evidence-based methods.
• Develop and update individualized care plans (manual therapy, gait, balance).
• Document care, observations, and interventions on a touch pad tablet.
• Educate clients and families to promote health, safety, independence.

🎯 Requirements

• Active PA Physical Therapist license.
• Home health experience preferred; training provided.
• Ability to travel to cases as assigned.
• Compassionate, strong assessment and communication skills.
• Strong written and verbal communication; team player.
