Physical Therapist- Home Health

📋 Description

• Make home visits to clients in designated territories.
• Assess mobility, ROM, transfers, balance, strength.
• Implement wound care with evidence-based dressings.
• Develop individualized PT care plans (manual therapy, gait training).
• Document care in EMR using touch-pad tablet; coordinate with nursing.
• Educate clients and families on rehab and independence.

🎯 Requirements

• AZ Physical Therapist license.
• Minimum 1 year PT experience.
• Graduation from an APTA-approved program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, problem-solving skills.
• Proficiency with EMR; prior EMR experience preferred.
