Physical Therapist- Home Health

📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests and measurements (ROM, gait, strength, balance, safety).
• Implement wound care with evidence-based dressings.
• Develop and update individualized care plans (manual therapy, gait, balance).
• Assess and revise care plans with nursing in multidisciplinary team.
• Educate clients and families on rehabilitative care for safety and independence.
• Document observations and interventions using a touch pad tablet.

🎯 Requirements

• A current New Hampshire Physical Therapist - PT license.
• A minimum of one year of recent work experience as a Physical Therapist - PT.
• Graduation from an APTA/CAAHEA-approved PT program, as indicated by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior EMR experience preferred.
