📋 Description

• Make home visits to clients in designated territories.
• Perform mobility tests, ROM, transfers, balance, and safety assessments.
• Implement wound care using evidence-based dressings.
• Develop and revise care plans, including manual therapy, gait, and balance.
• Educate clients and family members in rehabilitative care and safety.
• Document observations and interventions in EMR.

🎯 Requirements

• Current Vermont PT license.
• Minimum of one year of PT experience.
• Graduation from an APTA/CAHEA-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
