📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests: mobility, ROM, gait, strength, balance, safety.
• Provide wound care with evidence-based dressings and topical agents.
• Develop and update individualized PT care plans; include manual exercises and gait training.
• Educate clients and families on rehabilitative care for health and independence.
• Document observations and interventions using a touchpad tablet.

🎯 Requirements

• VA Physical Therapist license.
• Minimum one year PT experience.
• Graduate from an APTA/CAAHEA accredited PT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
