Physical Therapist - Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform mobility ROM, gait, balance, endurance assessments.
• Implement wound care using evidence-based dressings and topical agents.
• Develop and implement PT care plans including manual therapy, gait and balance training.
• Assess and revise care plan; coordinate with nursing in care plan.
• Educate clients and families on rehabilitative care and safety; document on EMR.

🎯 Requirements

• PA Physical Therapist (PT) license.
• Minimum 1 year PT experience.
• Graduation from an APTA- or AMA-accredited PT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational and problem-solving skills.
• Solid computer skills; EMR experience preferred.
