📋 Description

• Make home visits to clients in designated territories.
• Perform mobility, strength, balance, and safety assessments.
• Implement wound care and dressings using evidence-based methods.
• Develop and adjust individualized care plans with manual therapy and gait training.
• Revise care plans with nursing in the multidisciplinary team.
• Educate clients and families to promote health, safety, and independence.

🎯 Requirements

• New Hampshire PT license is required.
• Minimum 1 year of clinical PT experience.
• Graduation from an APTA-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
