Physical Therapist - Home Health

📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests: mobility, ROM, transfers, balance, safety.
• Implement wound care and protective interventions using evidence-based dressings.
• Develop individualized care plans, including manual therapy, gait training, balance.
• Educate clients and families in rehabilitative care to promote health and independence.
• Accurately document observations and interventions in EMR using a touch pad tablet.

🎯 Requirements

• A current NJ Physical Therapist license.
• Graduation from an APTA/CAAHEA-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior EMR experience preferred.
• One year of clinical PT experience preferred.
