Physical Therapist - Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform mobility tests and measurements, gait, balance, strength, and safety assessments.
• Implement wound care and dressings to prevent wounds and promote healing.
• Develop and implement individualized care plans with manual therapy, gait and balance training.
• Continuously assess and revise the PT care plan with the multidisciplinary team.
• Educate clients and families in rehabilitative care for safety and independence.

🎯 Requirements

• Current NJ PT license.
• At least 1 year of PT experience.
• Graduation from CAPTE-approved PT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficient with EMR systems (EMR experience preferred).
