📋 Description

• Make home visits to clients in designated territories.
• Assess mobility, ROM, gait, balance, and safety.
• Provide wound care and protective interventions.
• Develop and revise individualized care plans with PT interventions.
• Educate clients and families on rehab activities for independence.
• Document observations and interventions in the EMR.

🎯 Requirements

• Current MD PT license.
• Minimum 1 year PT experience.
• Graduate from an APTA/CAAHEA-approved PT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
