📋 Description

• Visit clients in designated territories.
• Assess mobility, ROM, balance, gait, strength, and safety.
• Implement wound care and evidence-based dressings as needed.
• Develop and update care plans with manual therapy and gait training.
• Educate clients and families on rehab activities and safety.
• Document observations and interventions in EMR.

🎯 Requirements

• Vermont PT license.
• Minimum of one year PT experience.
• Graduation from an APTA/CAPTE-accredited program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
