Implement wound care with evidence-based dressings and topical agents.
Develop and implement individualized PT care plans (manual therapy, gait, balance).
Assess and revise PT care plans; coordinate with nursing.
Educate clients and families; document care on a touchpad tablet.

🎯 Requirements

Active PA PT license.
Previous home health experience welcomed; training provided.
Ability to travel to cases as assigned.
Calm, reassuring presence; strong assessment and communication.
The ability to deliver patient care with compassion and reliability.
