📋 Description

• Make home visits to clients in designated territories.
• Conduct mobility, strength, balance, and safety assessments.
• Implement wound care and dressings per evidence-based practice.
• Create and adjust individualized PT care plans.
• Collaborate with nursing on the multidisciplinary care plan.
• Educate clients and families on rehab and safety.

🎯 Requirements

• Current Delaware Physical Therapist license.
• Minimum 1 year PT experience (preferred).
• Graduation from APTA/AMA-accredited program; transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
