Physical Therapist- Home Health

📋 Description

• Make home visits to clients in designated territories.
• Measure mobility, ROM, strength, balance, transfer status, safety.
• Implement wound care with evidence-based dressings and topical agents.
• Develop and update individualized care plans, including manual therapy, gait training, balance.
• Continually assess and revise the PT care plan, coordinating with nursing in the multidisciplinary team.
• Accurately document observations, interventions, and evaluations using an EMR tablet.

🎯 Requirements

• VA Physical Therapist license.
• Graduation from an APTA-approved PT program or CAAHEA-accredited program; provide transcripts/diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior EMR experience preferred.
