Physical Therapist- Home Health

📋 Description

• Make home visits to clients in designated territories.
• Assess mobility, ROM, gait, balance, strength, and safety.
• Develop and revise individualized PT care plans (manual therapy, gait, balance).
• Collaborate with nursing on multidisciplinary care plans and progress.
• Educate clients and families to promote health, safety, and independence.
• Document observations and interventions using EMR on a tablet.

🎯 Requirements

• Current NJ Physical Therapist license.
• One year of PT experience preferred but not required.
• Graduate from APTA-approved program or AMA-accredited school.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, problem-solving skills.
• Solid computer skills; EMR experience preferred.
