Physical Therapist - Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, ROM, gait, balance, safety).
• Implement wound care and dressings to promote wound repair.
• Develop and adjust PT care plans, including manual therapy, gait training, balance and other interventions.
• Educate clients and families on rehabilitative care and activities to promote health, safety, and independent living.
• Document observations and interventions in the EMR.

🎯 Requirements

• NJ Physical Therapist license.
• Graduation from an APTA-approved PT program.
• Independently manage time and work autonomously.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
• One year PT experience preferred.
