Physical Therapist- Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of mobility, strength, balance, and safety.
• Implement protective wound care using evidence-based dressings and topicals.
• Develop and implement individualized PT care plans, including manual therapy, gait and balance training.
• Continually assess and revise PT care plans; coordinate with nursing in the multidisciplinary plan.
• Educate clients and families in rehabilitative care to promote health and independence.

🎯 Requirements

• Current STATE PT license.
• Minimum one year of PT experience.
• Graduation from an APTA-approved PT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
• Florida Background Screening Clearinghouse.
