📋 Description

• Make home visits to clients in designated territories.
• Assess mobility, ROM, transfers, balance, and safety.
• Provide wound care and protective interventions to promote healing.
• Develop care plans with manual therapy and gait training.
• Collaborate with nursing on multidisciplinary plans; document care.
• Educate clients and families on rehabilitative care for independence.

🎯 Requirements

• Current North Carolina Physical Therapist license.
• Minimum of 1 year PT experience.
• Graduate from an APTA-approved program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
