Make home visits to clients in designated geographic territories.
Perform diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, ambulation patterns, strength and endurance, balance, and safety assessments.
Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence-based dressings.
Develop and implement individualized care plans, including manual exercises, gait training, balance, and other interventions.
Continually assess and revise the PT care plan, and participate with nursing in the multidisciplinary care plan.
Educate and instruct clients and families in rehabilitative care and activities to promote health and independence.
