- Make home visits to clients in designated territories.
- Perform diagnostic tests and measurements: mobility, ROM, strength, balance, and safety.
- Implement wound care and dressings; promote wound repair with evidence-based dressings and topical agents.
- Develop and implement individualized care plans, including manual therapy, gait training, balance, and other interventions.
- Continually assess and revise the PT care plan, and participate with nursing in the multidisciplinary plan.
- Educate clients and families in rehabilitative care and activities to promote health and safety.
- Accurately document observations and interventions using a tablet.
π― Requirements
- Active and current PT license
- One year of clinical experience preferred
- Home health care experience welcomed, but not required. We will train you!
- Ability to travel locally to cases as assigned
