📋 Description

• Sign-on bonus: $10,000; perform home visits in Hudson County, NJ.
• Provide home health PT to adult and geriatric clients on a per-visit basis.
• Evaluate mobility, strength, balance, gait, transfers, endurance, safety.
• Develop individualized treatment plans to improve function.
• Provide therapeutic exercise and gait training; educate patients.
• Document care using EMR; collaborate with multidisciplinary team.

🎯 Requirements

• Current New Jersey PT license.
• Graduate of an accredited Physical Therapy program.
• Strong communication, organizational, and clinical assessment skills.
• Ability to work independently with a flexible schedule.
• EMR experience preferred.
• Home health or geriatric care experience a plus.
