Provide home-based PT care from discharge to safe, independent living.
Perform mobility, strength, balance, gait, and safety assessments.
Develop and adjust individualized care plans with therapeutic exercises and gait training.
Collaborate with nursing to update the multidisciplinary care plan.
Utilize wound care techniques with evidence-based dressings and topicals.

🎯 Requirements

Active PA Physical Therapist license.
Home health experience preferred; training provided.
Ability to travel to assigned cases.
Calm, compassionate communicator with strong assessment skills.
Deliver patient care with compassion, excellence, and reliability.
