📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests: mobility, ROM, transfers, balance.
• Implement wound prevention and repair with evidence-based dressings.
• Develop and implement care plans: manual therapy, gait, balance.
• Continually assess and revise the PT care plan; collaborate with nursing.
• Educate clients and families in rehab for health and independence.

🎯 Requirements

• A current VA Physical Therapist (PT) license.
• Graduation from APTA-approved or CAAHEA-accredited program; transcript/diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, problem-solving skills.
• Solid computer skills; EMR experience preferred.
