Physical Therapist

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, strength, balance, safety).
• Implement wound care and protective interventions using evidence-based dressings.
• Develop and implement individualized care plans including manual therapy, gait and balance.
• Continuously assess and revise PT care plans with the nursing team.
• Educate clients and families in rehabilitative care for safety and independence.

🎯 Requirements

• Pennsylvania PT license required.
• Minimum one year recent PT experience.
• Graduation from APTA/CAHEA-approved program; transcript/diploma.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
