- Conduct PT assessments in facility and home to define care needs and goals
- Participate in IDT meetings to develop participant care plans
- Deliver and document interventions per care plans
- Provide active and passive exercise, gait training, ADLs
- Supervise Therapy Assistants and support FMP implementation
- Train staff, participants, and caregivers on therapeutic equipment
🎯 Requirements
- Bachelor’s degree in physical therapy; master’s/doctoral preferred
- Two years in clinical setting with frail/elderly population preferred
- Valid California PT license or proof of license eligibility
- CPR and First Aid certification within 180 days of hire preferred
